Randomized comparison of early supplemental oxygen versus ambient air in patients with confirmed myocardial infarction: Sex-related outcomes from DETO2X-AMI

چکیده
The purpose of this study is to investigate the impact oxygen therapy on cardiovascular outcomes in relation sex patients with confirmed myocardial infarction (MI). DETermination role Oxygen suspected Acute Myocardial Infarction trial randomized 6,629 at 6 L/min for 6-12 hours or ambient air. In present subgroup analysis including 5,010 (1,388 women and 3,622 men) MI, we report effect supplemental composite all-cause death, rehospitalization heart failure long-term follow-up, stratified according sex. Event rate endpoint was 18.1% allocated oxygen, compared 21.4% air (hazard ratio [HR] 0.83, 95% confidence interval [CI] 0.65-1.05). men, incidence 13.6% 13.3% (HR 1.03, CI 0.86-1.23). No significant interaction found (P= .16). Irrespective treatment, occurred more often men (19.7 vs 13.4%, HR 1.51; CI, 1.30-1.75). After adjustment age alone, there no difference between sexes 1.06, 0.91-1.24), which remained consistent after multivariate adjustment. normoxemic MI did not significantly affect mortality men. observed worse outcome explained by differences baseline characteristics, especially age.

A randomized controlled trial of supplemental oxygen versus air in cancer patients with dyspnea.
CONTEXT The symptomatic benefits of oxygen in patients with cancer who have nonhypoxic dyspnea are not well defined. OBJECTIVE To determine whether or not oxygen is more effective than air in decreasing dyspnea and fatigue and increasing distance walked during a 6-minute walk test. PATIENTS AND METHODS Patients with advanced cancer who had no severe hypoxemia (i.e., had an O2 saturation lev...

Background and Objective: One of the most important advancements regarding the care of patients with acute myocardial infarction is the administration of anti-coagulation medicines (e.g., streptokinase). However, it must be noticed that this medicine requires rapid and timely administration. Moreover, Percutaneous Coronary Intervention (PCI) is increasingly used as a method of revascularization...

BACKGROUND The clinical effect of routine oxygen therapy in patients with suspected acute myocardial infarction who do not have hypoxemia at baseline is uncertain. METHODS In this registry-based randomized clinical trial, we used nationwide Swedish registries for patient enrollment and data collection. Patients with suspected myocardial infarction and an oxygen saturation of 90% or higher wer...
متن کاملHEART RATE: A PREDICTOR OF EARLY MORTALITY IN PATIENTS WITH MYOCARDIAL INFARCTION
A number of epidemiologic studies have reported a positive relationship between heart rate, cardiovascular disease and mortality. To examine the correlation between heart rate and mortality after acute myocardial infarction (AMI), 2147 patients hospitalized in coronary care units in Isfahan were investigated in a cross-sectional study. Their heart rate was measured according to an electroca...
متن کاملA randomized controlled trial of oxygen therapy in acute myocardial infarction Air Verses Oxygen In myocarDial infarction study (AVOID Study).
BACKGROUND The role of routine supplemental oxygen for patients with uncomplicated acute myocardial infarction (AMI) has recently been questioned. There is conflicting data on the possible effects of hyperoxia on ischemic myocardium. The few clinical trials examining the role of oxygen in AMI were performed prior to the modern approach of emergent reperfusion and advanced medical management. ...
